Community Mental Health
The Community Mental Health program provides psychiatric assessment and treatment, consultation, and rehabilitation/case management. A multidisciplinary team of clinicians and psychiatrists provide the mental health care. As the program is accessible to those primarily marginalized by language, race and culture, language specific services are available to the Portuguese, Spanish, Italian, Cantonese and Mandarin-speaking communities. Interpreter services are available for other language groups. The program has a strong teaching component and offers training to residents and students of various professional disciplines.
Goals
- To promote and improve the quality of life of individuals affected by mental health problems.
- To promote accessible, comprehensive mental health services to those marginalized by language, race and culture.
- To support community care providers in the provision of mental health care through the shared care model of practice.
- To share our expertise through teaching and learning opportunities to residents and students of various health care disciplines.
Services
- Psychiatric assessment, consultation and follow-up, brief psychotherapy, group programs, psychiatric rehabilitation/case management, and an injection clinic
- Mindfulness meditation and cognitive behavioural treatment of anxiety
- Family education and support for Portuguese, Spanish and Italian-speaking families
- Toronto Urban Health Alliance: on-site mental health services to Community Health Centres utilizing a shared care practice model
- Out-patient department/Resident Teaching Program
Eligibility
Individuals 16 years of age and over with a mental health problem or concern.
