We are targeting an average appointment time of less than three weeks for routine referral and less than 48 hours for urgent referrals.

In order to make a referral, we ask that:

1. You call us at 1-888-UHN-HART (846-4278) during operating hours of 0830-1600 hours Monday through Friday to speak with intake staff.
2. Voicemail will be retrieved and returned by the next business day.
3. Complete the referral form to request the full information required to facilitate a timely visit which would include booking any other necessary testing without duplication.
4. Fax any tests, which are relevant at (416) 340-4127.
5. We have created this clinic to make obtaining a cardiology consult easier; however, you should feel free to contact any UHN cardiologist's office directly if that is the route you prefer.
