At UHN, patient safety is always our priority. Recently, a Patient Safety Council has been formed to coordinate, supervise and help implement our patient safety initiatives. The Council is a multidisciplinary leardership group that has representation from Medicine, Surgery, Nursing, Pharmacy, Medical Engineering, Risk Management, and Senior Management. The Council has developed the following list of actions to be taken and is fostering implementation and monitoring progress.
- Establishment of a culture where the reporting of adverse medical events and near misses is supported and facilitated;
- Implementation of physician computer order entry, emphasizing medication orders;
- Support and expansion of human factors analysis for interaction of staff with equipment;
- Participation in provincial and national patient safety councils and initiatives;
- Further development of electronic reporting and review mechanisms which facilitate quality improvement.
