| • Assess a patient's ability to move within his or her environment |
| • Help maximize physical safety and plan treatment to help patients exercise and improve breathing, strength, movement, and activity |
| • Actively engage patients in planning and carrying out treatment |
| • Educate patients about their conditions |
| • Plan safe discharge, including suggestions for how patients can continue with their own activity plans and exercises |
| • Help link patients with community resources or rehabilitation institutions when needed |
